Before filling out the required documents, ensure that you have all the information you will need to complete them. For reference purposes, you may want to have a copy of your 2A, PHA record, and current record APFT as well as your most current HIV test results. Note that if applicable, you will also need your DA Form 5500/5501. The previously listed documents are not included in your packet when it is submitted, but must be checked to ensure that you are qualified for entry into the course.
